To gain an appointment, please contact us directly.  The prospective producer should provide a short history of your agency, along with information as to how many Benefit Producers and how many Property/Casualty producers are employed.  Provide us with the full name of your agency, along with the name of the owning entity if your agency uses a DBA, the full street address, your mailing address in addition if you choose to have it utilized, your telephone number, facsimile number and email addresses for the producers involved (mobile numbers optional).

We require that you provide copies of your agency licenses for whichever States you desire to do business, along with a copy of your Errors & Omissions Policy Declaration Page.
