FOR General Inquiries & Questions

If a plan participant has general questions regarding Plans and Benefits, or if the participant has questions regarding either enrollment or eligibility, or if the participant has questions regarding the state of the employer's account (including premiums and payments), or if the participant requires assistance regarding service from the provider or drug networks, or if the participant requires a replacement Employee Identification Card (lost or misplaced), then the participant is encouraged to contact AARBIS, the Program Administrator, toll free at 866-604-6445. Office hours are 9-5 Pacific Standard Time, Monday through Friday (except national holidays). The participant can also go online and email AARBIS at customerservice @aarbis.com.

FOR PPO Network Inquiries

If a plan participant has a question regarding the PPO Network provider, hospital or facility, or requires help to find an appropriate doctor, hospital or facility that will meet the participant's healthcare needs, the participant should contact the provider network directly at the toll free number, or web address contained on their ID card and in their Employee Guide.

FOR Claim or Coverage Inquiries

If a plan participant desires to inquire regarding whether the participant has a specific coverage, or exactly what is covered under a benefit coverage, or exactly what process to follow in making a claim, or has an inquiry regarding the applicable dollar benefit, or has an inquiry regarding specific claim payments, then the participant should contact AIG Claims Office at its toll free number (877) 503-9095. The participant may also FAX claims at toll free (877) 514-5206.

FOR Prescription Drug Inquiries

If a plan participant desires information regarding how to use the Walgreen's Drug Card program, including: locating a participating pharmacy; mail order and Advantage90 programs; generic or lower cost substitutes that may be available, and PlatinumPLUS discounted drug costs, the participant should call Walgreen's Health Initiative at its toll free number (800) 451-3103, or go online at www.mywhi.com.

NOTE:  If for any reason your or the Employer is dissatisfied with the response of WHI, either of the PPOs or AIG Claims, then AARBIS should be notified immediately.
