PlatinumPlus - Claims

A claim form and instructions are included with the PlatinumPLUS Employee Welcoming Kits.  Forms are also available through the Program Administrator, AARBIS, by email at customerservice@aarbis.com.  You may also print forms through this site.  Claim forms should also be available from the employer’s Employee Benefits or Human Resources department.

Plan participants can choose to assign or not assign benefits to a provider or facility.

Assigning benefits means the employee is authorizing the Insurance Company to pay the insurance benefits directly to the medical provider.

When benefits are not assigned to the provider benefits are paid directly to the Employee.  

The required payment procedures of medical providers can vary greatly.  Some providers may require all or a partial payment at the time medical services are provided, while others may not.

Filing a Claim and Assigning Benefits
 
If you elect to assign benefits to provider:  Upon arrival, the employee should give the provider a Claim Form that is both signed by the employee with a check placed in the box authorizing assignment.  If the employee is utilizing a PPO network provider, the employee should also present them with the Employee ID card, which indicates the PPO network information.
 
The provider will submit an itemized bill and claim form to the AIG Claims Office.  The employee may still be required to make a payment based on the benefits payable and the difference between the total charges (or discounted charges if a PPO provider) and the scheduled benefit claim payment to provider.

Filing a Claim and Not Assigning Benefits 

If the employee chooses not to assign benefits directly to the provider:  At the conclusion of the visit, the office staff will bill (and collect), less the appropriate Network discount, the full amount of the medical charges.  The employee should forward an itemized bill for the services rendered, along with a completed and signed claim form to the AIG Claims Service Office:
